The memory care wing is secured to ensure each tenant’s safety. There are 19 memory support suites with simplified floor plans that offer a private room and bathroom. With secured exits and a courtyard, we are able to offer peace of mind to loved ones, and the flexibility for tenants to move freely indoors and out. Families and loved ones are encouraged to furnish the rooms with simple, safe and familiar items.

To provide the best level of oversight, all packages will include:

  • RN initial and on-going assessments
  • Professional nurse (LPN or RN) coordination with physician, pharmacy, professional therapy staff and family care conferences
  • Assistance arranging transportation and appointments
  • Daily well-being checks
  • Supervision in dining room
  • Accommodations for special dietary needs
  • Personal supplies which include: toilet paper, paper towels, hand soap, (2) trash cans, trash bags, lotion, shampoo, deodorant, razors, denture care supplies, toothbrush, toothpaste, and body wash


Functions in this service level will include supervision, stand-by assistance, prompting and reminders to complete tasks. No hands-on assistance or minimal hands on experience at this level.

  • Simple medication management, which may include storing, ordering, setting up and administering of medication

Functions in this service level will include supervision and stand-by or hands-on assistance of one to two staff persons or a mechanical device to complete tasks.

  • Dressing and grooming
  • Bathroom assistance
  • Shower/bath assistance as defined above
  • Assistance of up to one person with transfers and ambulation
  • Assist of one to propel wheelchair
  • Simple medical monitoring, which may include blood pressure, pulse, or weight or other interventions deemed necessary by RN or MD
  • Simple and supportive reminders
  • Consistent assistance with eating (each meal, each day)
  • Medication management, which may include storing, ordering, setting up, and administering medications; assisting with blood sugar monitoring and insulin injections up to two times per day
  • Other tasks determine by the RN to appropriately fit at this level of staff time and training


Level 3 is ideal for the individual who needs the services explained in Level 1 and 2 and also require a behavioral management plan to manage daily behaviors. Behavior management plans at this level would include frequent redirection and behavior modification that goes beyond supportive reminders and reassurance.
Behavior Modification
Level 3 is appropriate when behavior modifications become frequent redirection throughout the day and go beyond supportive reminders and reassurance. Redirection will be given if and when needed to support the client and their specific behavior. The amount of redirection needed significantly expands the amount of time needed to accomplish the activities of daily living and could involve more than one staff person.

  • Dressing and grooming
  • Bathroom assistance as defined above
  • Assist of two or mechanical lift for transferring and ambulation
  • Shower/bath assistance as defined above
  • Turning and repositioning for preventive skin care
  • Complex medical management includes all services listed at Level 2, and assistance with devices such as catheter, ostomy, oxygen or nebulizer
  • Frequent behavior modification
  • Frequent reassurance checks
  • Complex and/or consistent behavior modification and redirection
  • Complex medication management which may include blood sugar monitoring, insulin injections up to 4x per day and/or sliding scale insulin and regularly scheduled non-insulin injections
  • Other tasks determined by the RN to appropriately fit at this level of staff time and training
